Root cause failure analysis (RCFA) is the process of investigating the fundamental, underlying cause of the failure of a process or piece of equipment and using that information to implement a solution to prevent the failure from happening again. The goal is to provide a long-term solution that addresses the root cause of the problem, rather than address the symptoms and allow the problem to recur. This course discusses the different stages of RCFA, the difference between “root causes” and “causal factors,” and several tools and methods used in RCFA. It also includes an example scenario to provide some real-life context.
Course Objectives

By the end of this course, you will be able to:

  • List the key steps in a Root Cause Failure Analysis (RCFA)
  • Distinguish between “root causes” and “causal factors”
  • Explain the tools and methodologies that can be used for an RCFA, including the Five Whys, Fault Tree Analysis, Fishbone Diagrams, Failure Mode and Effects Analysis, Kepner-Tregoe Analysis, and Five Ps
  • Describe critical operator contributions to an RCFA
  • Identify when an RCFA is appropriate
